    It's like there was a conspiracy amongst the drug naming people to produce EXACTLY this mnemonic. If Metformin (the biguanide) doesn't work, I'll have no problem remembering the other drug classes! .

    You mentioned that GLUT-2 transporters are in the Renal tubules, for me this is counter-intuitive. GLUT-2 transporters have a high Km (low affinity for Glucose) therefore the transfer of Glucose is less likely in low to normal concentrations. However one of the functions of the Renal Tubules are to reabsorb Glucose as quickly as possible. So why would GLUT-2 be present in the Renal Tubules where GLUT-3 would make more logical sense. . this man is for real :) glad to have someone like him while i am studing for my steps, i bought his 400 hours lecture videos, i think he is by far better then kaplan, dr goljan, pass program... or even my med school :P LOL . An amazing lecturer. Learned more about antidiabetic drug mechanism of action in 45 minutes with Dr. Najeeb than I learned in 5 years of undergrad and 4 years of grad school. .

However, it is actually the opposite. Increase ICF glucose cause the shut down of K+ channels => depolarize the cells.     Great questions Nik, taking coumadin w/ an oral hypoglycemic agent is contraindicated because it may cause hypoglycemia in your patient. Both medications working together is bad for the patient. Great observation about steroids raising a patient's blood glucose level I am so proud of you for knowing that, most new nurses don't! However you should note that corticosteroids sometimes inhibit the metabolism of the oral hypoglycemic agent so subcutaneous insulin injections are a better combination..
    Is Byetta administered orally My drug ref book states it's injected subq. And my pharm textbook doesn't have it categorized as an oral hypoglycemic. I'm confused. :(.

    Given that studies consistently observe an increase in mortality in type 2 diabetes patients treated with insulin compared to other diabetes medications, I think it is quite reasonable to delay the use of insulin therapy as long as possible..
    I am amazed at all the stupid and noxious comments written here by people who thrive on conspiracy theories. This is not a talk on "management of diabetes", but one about the pharmacological treatment, with a focus on the drugs available. It is very clear from the beginning of the lecture that pharmacological treatment is not the first line, but something that should be offered to patients who do not achieve their goals or cannot tolerate the lifestyle changes proposed. Lifestyle changes and dietary reeducation are still the mainstay for hyperglycemia control (and scientists acknowledge this fact). Keep this place free from stupid, paranoid references to "Big Pharma", please. Without "Big Pharma", you could have already died from disseminated gonococcal infection..
    Lifestyle changes are the first line treatment and the first thing all doctors recommend to their patients. Unfortunately most people are not willing to do too much for their health, don't exercise or eat healthy. And you need to help these people somehow too..
